I only really omit the gg if I'm really angry/disappointed in my play. If I make a lot of stupid decisions or play awfully, I just leave in frustration.

I gg almost all the time, but if I go full retard and instantly lose to my own error, I might just quit in disgust and re-queue.
Interesting how sc2 players seem to be so ready to hate themselves compared to how many other competitive games (CS, dota) are more interested in blaming team mates/opponents.
I always "GL HF" out of respect to the other player; it only takes a few seconds to do and to be quite honest; it's not like doing-so hinders your game-play as there's not much else to do in those first few seconds of the game (other than spam APM, of course ).
And I always end the game with a "GG" and/or "GG WP" - regardless of how I lost; because at the end of the day; SCII is a game where I am solely responsibly for my losses & wins and therefore whether I was beaten via cheese or in a long-macro game is irrelevant; as it clearly means I was out-played.
The bit above I mentioned about solely being responsible for my own wins/losses is such an amazing factor that I about SCII, but that's another story! Although I often don't receive "GL HF" in return; or in some cases a "GG" but it's always nice when I do, even nicer when people have a quick-chat after the game...
If I lose to cheese or a massive ball of muta's I don't gg generally.
Any other situation I do. I try to say glhf at the start and talk a little so I'm less annoyed if I lose. Easier to say gg If you've had a bit of friendly chat. But it depends on whether it IS a good game or not.

Not necessarily good, but interesting.
No surprise, those are team games. When a team fails, people try to assign blame, and rage out. Plus, it's harder to measure individual skill, which promotes looking outward for reasons.
I GG, IF im happy with how i played, otherwise; cheese, allin's, i do something which costs me the game, i won't, oh and 2v2, 3v3, 4v4 i won't GG, unless the game was like aweosmely epic.
Otherwise im quiet apart from the opening glhf. if they BM me and start talking shit, making comments, i'll serve it back, otherwise i'll leave as soon as i feel i've lost a game.
I always gg... and glhf at the start... its pretty automatic... i kind of figure that if i got cheesed i should have stopped it and if i lose later on in the game then its more than i just got outplayed

An unashamed plug for myself.
Quitting without a GG can often be giving the other player exactly what they want. The knowledge that they got to you, made you mad etc can be fulfilling for them if they BM'ed or cheesed you. So I've started to force myself to say it in those situations just to deny them that satisfaction
