People who play Riki, and only go in for the last hits using blink strike.
Same for bounty hunter and the glaive.

People who play an initiator role, yet don't initiate - The same goes for nukers that initiate and get their shit slapped.

When mid doesn't call miss, and then all of a sudden a Pudge 3 levels higher than you with double damage has you in his hook.

I don't think I've been as frustrated and angry at anyone as much as I have when playing dota.
But goddamn it's fun.
Edit: we should all start a team of non-terrible people.
I'll just go with everyone on your team picking stuff like bloodseeker and ursa and riki and getting super cocky and bm after a few ganks until they get to a certain point in the game where they realise they have no real team fight potential, then BM the rest of the team for being horrible.
Yeah those games are bad. I even keep asking for courier/wards, but it's useless.
One game I constantly berated CM to buy wards. "Where's the wards CM?" "Get some ******* wards CM." etc. She never got them. IIRC one of her reasons was along the lines of "You all die too much no point in wards."

Boots with first 600 gold gives no survivability, and no help to last-hitting, sometimes it can be handy if you have a specific strat tog et first blood, in which case you'll gian enough for it to be worth it, but in general its "frowned upon" because you're skipping a natural progression of items.
Think nexus first, but with 0 defense, or something, it can work, it can be awesome, but usually its better to use a forge and cannon at some point

The best is when I go PA mid doing pretty well on my last hits and we have a BH/Riki lane the Riki then proceeded to go 0-5 within the first five minutes and doesnt even have level 6 by the time im around level 15. So what does he do? bitch for the rest of the game about how we're a bunch of noobs who dont know shit about dota. I somehow manged to pull the game back with my friend who was Drow Ranger bot but it wasnt an easy or enjoyable match.

It kinda cool this MM system..everytime I go on a winning streak, it says here's your reward, play with people who feed 20 times in 20 minutes, or go afk at tower and say "lolzzz I'm eating my breakfast.". In the case of the breakfast guy, he was better off afk, because he ended up feeding after he came back.
